Resolution

Setup a standard Sales Order Import with the following fields. The key is to use * for the CRM Opportunity ID. This is an example with Demo Data:

Process import in Sales Order Entry and Post. The proper records will be created in MAS_System.CU_OrderQuoteDetail and CRM_OrderItem tables.

Note: Do not change the Customer Number to another customer from inside the Sales Order. Once that is done it breaks the link to CRM.
